All co-pays and non-covered charges are due at the time of your visit including Refraction fees. A Refraction is an examination to correct your eyesight to the best possible vision. It is generally not covered by insurance and therefore is due at the conclusion of your visit.
We accept CASH, CHECKS, MASTERCARD, VISA, DISCOVER, and AMERICAN EXPRESS at the time of your visit.
IMPORTANT-All unpaid co-pays are subject to a penalty.
Kindly give 24 hours notice from your scheduled appointment time if you need to cancel or reschedule your appointment. Failure to show up for your appointment or not canceling 24 hours in advance will result in a $50.00 charge.
We contract with insurance companies as a medical provider by providing medical eye care to our patients. If you have a vision rider or routine eye exam provision with your medical insurance you may qualify for routine eye exam coverage with our doctors.
Also, we believe that our patients are educated patients and take the time to read their health plan benefits to know their coverage. Please take the time to review your policy prior to calling us. Thank you!!
If you have requested a ROUTINE VISION EXAMINATION; the diagnose on your bill will correspond with your appointment and will be coded as a ROUTINE VISION EXAMINATION. This diagnoses cannot be changed. If your insurance company denies your visit because they don't cover routine exams, you will be responsible.
