Billing & Insurance

Co-pays and Payment Policy

All Copays and non-covered charges are due at the time of your visit.

We accept all major credit cards, Cash and Checks.

IMPORTANT: All unpaid co-pays are subject to a copay penalty.

Referral Plans

If your plan requires a referral from your primary care provider; you will be responsible for obtaining one prior to your appointment. If you do not have your referral at the time of your appointment, Connecticut Eye Consultants reserves the right to reschedule.

Cancellations

Please give 24 hours advance notice for cancellation of your appointment. Failure to notify the office will result in a $75 charge.

Insurance Coverage

Connecticut Eye Consultants participates in most major insurance plans. PLEASE CONTACT YOUR INSURANCE CARRIER FOR CURRENT PARTICIPATION STATUS.

We Do Not Participate With:

We provide medical eye care to our patients. If you have a vision rider or well vision exam provision with your medical insurance, you may qualify for a well vision eye exam with our doctors.

Well Vision Examination – a general examination of the eyes which does not address ocular or medical complaints or symptoms. A separate appointment will have to be scheduled to address those concerns.

Refraction – an examination to correct your eyesight to the best possible vision and generate your glass or contact lens prescription may not be covered by your insurance.

We believe our patients are well educated patients and have read their insurance benefits prior to their visit. Please make sure you understand your insurance policy limitations prior to making an appointment with us. 

If you have requested a well vision examination, the diagnosis on your bill will correspond with your appointment and will be coded as a well vision examination. The diagnosis cannot be changed. If your insurance company denies your visit because your policy does not cover well vision exams, you will be responsible for the bill.
